Organizational Culture & Compliance


• Adheres to the division's policies, procedures and standards while ensuring compliance with applicable regulatory bodies.

• Contributes towards an innovative culture of continuous improvement for enhancing operational efficiency and effectiveness.

• Participates in meetings and maintains professionalism and confidentiality as per the organization's standard code of conduct.

• Completes personal performance evaluation cycle in a timely manner.

Clinical Practice


• Assists in the effective implementation and evaluation of the resident/patient’s care is in collaboration with the family and in accordance with the organization's policy and procedures and scope of service.

• Demonstrates accountability for own professional judgment, actions, outcomes of care and continued competence.

• Recognizes the boundaries of their scope of practice and limits of own competence.

• Under supervision utilizes up to date evidence-based practice in the provision of resident/patient & family-centered care.

• Under nursing supervision works in collaboration with the interdisciplinary team to deliver individualized care from point of the first contact throughout the episode of care.

• Reports all significant changes/deterioration with resident/patient conditions to the registered clinician, initiates appropriate action within the scope of practice.

• Supports clinical staff in the prioritization of the resident/patient care.

• Always demonstrates service excellence and advocacy to support the resident/patient’s rights and dignity.

• Take all reasonable steps to protect residents who are vulnerable or at risk from harm, neglect or abuse and escalates concerns to nursing staff.

• Promotes quality of life and embraces the unique social and cultural role of the family.

• Facilitates a climate of mutual understanding, and appreciation of cross-cultural differences inherent in multicultural society.

• Under the direction of the Registered nurse or registered clinician assist in the activities of daily living for patients in accordance with the patient care plan e.g. distribution of patient meals and assisting patients with hygiene needs.

• Responds to all levels of emergencies according to the organization's policies and procedures and within own scope of practice.

• Demonstrates a working knowledge of resource consumption and practices within a cost-effective framework.




Education


• Assists in the provision of education for residents/patients and families.

• Assists in the provision of a suitable environment that supports resident/patient and family learning.

• Assists in ensuring the resident/patient and family are provided with comprehensive information and resources to meet their health care needs.

• Assists in health promotion activities in the healthcare organization and community.

• Maintains confidentiality and security of a resident/patient, family, and the organization information in accordance with the confidentiality policy.

• Facilitates effective communication between the resident/patient, family and the interdisciplinary team as required.

• Under supervision/direction, liaises with other health care providers as necessary to ensure appropriate and timely interventions for the resident/patient.

• Provides comprehensive handover to ensure resident/patient safety.

Diploma in relevant field




Job skills:


Fluent in written and spoken English
